The form-to-email provider processes your submission only to deliver it to us.

Information that becomes part of a patient's medical record is retained under the separate rules that govern health records.

No website or method of transmission over the internet is completely secure, so we cannot guarantee absolute security. Please do not send sensitive medical details or emergency information through our forms.

Our website is intended for adults, including patients, families, caregivers, referring providers, and job applicants. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13, consistent with the Children's Online Privacy Protection Act (COPPA). Do Not Track

Some browsers offer a "Do Not Track" setting. Because there is no common industry standard for how to respond to these signals, our website does not currently respond to Do Not Track signals.
